Use a screen reader to create a new form in Microsoft Forms

This article is for people who use a screen reader program such as Windows Narrator, JAWS, or NVDA with Microsoft 365 products. Use Microsoft Forms with your keyboard and a screen reader to create a new form. We have tested it with Narrator in Microsoft Edge and JAWS and NVDA in Chrome, but it might work with other screen readers and web browsers as long as they follow common accessibility standards and techniques. You'll learn how to name the form, add questions to it, and share the form to get responses.

Create a new form

  1. On the Microsoft Forms main page, press the Tab key or Shift+Tab until you hear "New form, button," and then press Enter. You hear: "Untitled form." If you hear "Button" instead, press Enter to close the dialog box before continuing.

  2. To add a title to the form, press the Tab key until you hear "Untitled form, button," and then press Enter. Type a name for the form.

  3. To add a description for the form, press the Tab key until you hear "Form description," and then type the form description.

  4. To add a question to the form, press the Tab key until you hear: "Add new question or section, button, collapsed," and then press Enter. The focus moves to the first option, "Choice."

  5. To select a question type, do one of the following:

    • To select the Choice question, simply press Enter.

    • To select the Text, Rating, or Date question, press the Tab key until you hear the type you want, and then press Enter.

    • To access the additional options at the end of the Add new menu, press the Tab key until you hear "More question types, button, collapsed," and then press Enter. With Narrator, press the SR key+Right or Left arrow key until you hear the option you want. With JAWS and NVDA, press the Down or Up arrow key instead. Press Enter to select an option.

  6. You hear: "Question title." Type the question.

  7. If you selected the Choice question, add the first two response options to the question. Press the Tab key until you hear "Enter a name for this option," and then type the option text. Repeat for the second option. To add additional response options, press the Tab key until you hear "Add option," press Enter, and then add the option text as instructed above.

  8. To customize the questions, you can do one or more of the following:

    • For the Choice question, to allow respondents to select multiple response options, press the Tab key until you hear "Multiple answers," and then press Spacebar.

    • For the Text question, to allow respondents to enter multiple lines of text, press the Tab key until you hear "Long answer," and then press Spacebar.

    • To make answering a question mandatory, press the Tab key until you hear "Required," and then press Spacebar.

  9. To finish editing a question, press Esc. The question is saved in your form.

  10. To add additional questions, press the Tab key until you hear "Add new question or section, button, collapsed," and then press Enter. Then repeat the steps from step 5 onward as needed.

Preview a form

Before you share the form with the respondents, check the form in the preview. You can even complete the form yourself and make sure that everything works as it should.

  1. In the form you want to check, press Shift+Tab until you hear "Preview," and then press Enter.

  2. Press the Tab key or Shift+Tab to navigate the fields in the form preview.

  3. To test the form yourself, respond to the questions and submit the form as instructed in Use a screen reader to respond to form or quiz questions in Microsoft Forms.

  4. To exit the preview, press Shift+Tab until you hear "Back, button," and then press Enter.

Share a form to get responses

When your form is ready, share a link to the form with the respondents.

  1. In the form you want to share, press Shift+Tab until you hear "Collect responses," and then press Enter. You hear: "Copy link, button."

  2. To select who can respond to your form, press the Tab key until you hear "Close," and then press the Tab key once more. You hear the current response option, for example, "Only people in my organization can respond." With Narrator, press the SR key+Left or Right arrow key until you hear the option you want, followed by "Radio button." With JAWS and NVDA, press the Up or Down arrow key instead. Press Spacebar to select the response option.

  3. Press the Tab key until you hear "Copy link, button," and then press Enter. The link to the form is copied to the clipboard.

  4. Paste the link, for example, to a Microsoft Teams channel or an email.

Create a new form

  1. On the Microsoft Forms main page, press the Tab key until you hear "New form," and then press Return.

  2. Press Control+Option+Left arrow key until you hear "Form title," and then press Control+Option+Spacebar to edit the form title. Type a name for the form.

  3. To add a subtitle for the form, press the Tab key twice. You hear: "Form description." Type a subtitle for the form.

  4. To finish editing the form title, press Esc.

  5. To add a question to the form, press the Tab key until you hear "Add new," and then press Return. You hear: "Choice."

  6. To select a question type, do one of the following:

    • To select the Choice question, press Return.

    • To select the Text, Rating, or Date question, press the Tab key until you hear the type you want, and then press Return.

    • To access the additional options at the end of the Add new menu, press the Tab key until you hear "More question types," and then press Return. To browse the options, use the Down and Up arrow keys, and then press Return to select an option.

  7. You hear: "Question title." Type the question.

  8. If you selected the Choice question, add the first two options to the question. Press the Tab key until you hear "Choice option text," and type the option text. Repeat for the second option. To add additional options, press the Tab key until you hear "Add option," press Return, and add an option text as instructed above.

  9. To customize the questions, you can do one or more of the following:

    • For the Choice question, to allow respondents to select multiple options, press the Tab key until you hear "Multiple answers," and then press Spacebar.

    • For the Text question, to allow respondents to enter multiple lines of text, press the Tab key until you hear "Long answer," and then press Spacebar.

    • To make answering a question mandatory, press the Tab key until you hear "Required," and then press Spacebar.

  10. To finish editing a question, press Esc. You can now add additional questions by repeating these instructions starting at step 5.

Preview a form

Before you share the form with the respondents, check the form in the preview. You can even complete the form yourself and make sure that everything works as it should.

  1. In the form you want to check, press Shift+Tab until you hear "Preview," and then press Return.

  2. Press the Tab key or Shift+Tab to navigate the fields in the form preview.

  3. To test the form yourself, respond to the questions and submit the form as instructed in Use a screen reader to respond to form or quiz questions in Microsoft Forms.

  4. To exit the preview, press Shift+Tab until you hear "Back, button," and then press Return.

Create a new form

  1. On the Microsoft Forms main page, swipe left or right until you hear "New form, button," and then double-tap the screen. You hear: "Form title, untitled form, button."

  2. To add the form title, double-tap the screen, swipe left once, and then swipe right once. You hear: "Text field." Double-tap the screen, and then use the on-screen keyboard to type the title. When you're done, swipe left until you hear "Done, button," and then double-tap the screen.

  3. To add a description for the form, swipe right until you hear "Form description," double-tap the screen, and then use the on-screen keyboard to type the description. When you're done, swipe left until you hear "Done, button," and then double-tap the screen.

  4. To add a question to the form, swipe right until you hear "Add new question or section, button," and then double-tap the screen.

  5. To select a question type, do one of the following:

    • To select the Choice question, double-tap the screen.

    • To select the Text, RatingDate, Ranking, or Likert question, swipe right until you hear the type you want, and then double-tap the screen.

  6. You hear: "Question title." Double-tap the screen, and then use the on-screen keyboard to delete the default text and type the question. Swipe left until you hear "Done, button," and then double-tap the screen.

  7. If you selected the Choice question, add the first two options to the question. Swipe right until you hear "Choice option text," and then double-tap the screen. Use the on-screen keyboard to delete the default text and type the option text. Swipe left until you hear "Done, button," and then double-tap the screen. Repeat for the second option. To add additional options to the question, swipe right until you hear "Add option, button," double-tap the screen, and then add the option text as instructed above.

  8. To customize the questions, you can do one or more of the following:

    • For the Choice question, to allow respondents to select multiple options, swipe right until you hear "Multiple answers, checkbox," and then double-tap the screen.

    • For the Text question, to allow respondents to enter multiple lines of text, swipe right until you hear "Long answer, checkbox," and then double-tap the screen.

    • To make answering a question mandatory, swipe right until you hear "Required, checkbox," and then double-tap the screen.

  9. To add additional questions, swipe right until you hear "Add new question or section, button," and then double-tap the screen. Repeat steps 5 to 8 for each new question.

Your form is saved automatically after each change you make, so you don't need to save it when you're done.

Preview a form

Before you share the form with the respondents, check the form in the preview. You can even complete the form yourself and make sure that everything works as it should.

You can preview a form immediately after finishing it as described in Create a new form, or you can open a previous form from the Microsoft Forms main page.

  1. In the form you want to check, tap at the top of the screen with four fingers, swipe right until you hear "Preview, button," and then double-tap the screen.

  2. Swipe right to navigate the fields in the form preview.

  3. To exit the preview, tap at the top of the screen with four fingers. You hear: "Back, button." Double-tap the screen.

Share a form to get responses

When your form is ready, share a link to the form with the respondents.

You can share a form immediately after finishing it as described in Create a new form, or you can open a previous form from the Microsoft Forms main page.

  1. In the form you want to share to get responses, swipe right until you hear "Share, button," and then double-tap the screen.

  2. To select who can respond to your form, tap at the top of the screen with four fingers, and then swipe right until you hear "People to share with," followed by the current sharing permission option. If you want to change the option, double-tap the screen. Swipe right or left until you hear the option you want, and then double-tap the screen to select. To return to the sharing view, swipe left until you hear "Back, button," and then double-tap the screen.

  3. Swipe right until you hear "Copy link, button," and then double-tap the screen. The link to the form is copied to the clipboard.

  4. Paste the link, for example, to a Microsoft Teams channel or an email message.

  1. On the Microsoft Forms main page, swipe left or right until you hear "New form, button," and then double-tap the screen.

  9. To add additional questions, swipe right until you hear "Add new question or section, button," and then double-tap the screen. Repeat steps 5 to 8 for each new question.
